Clark Camp Forms

There are five forms below that need to be filled out in order to finish registering for camp. "Release of Liability, Waiver of Right to Sue, Assumption of Rick", Sports-Related Concussion and Head Injury Fact Sheet and Parent/ Guardian Acknowledgement Form",  "Walker/ Pick Up Form", "Photo/ Video Release" and "Medical Form/Emergency Contact Form". All forms need to be read, acknowledged and signed in order to register for camp. You will need to print, acknowledge and sign the "Medical Form/ Emergency Contact Form" and bring that with you the first day of camp.

Camp Code of Conduct

Campers are expected:

1.) To respect the camp director, staff, and other campers.

2.) To respect the equipment and facility.

3.) To not climb or move the nets, goals or other equipment.

4.) To report any sign of illness or injury to a staff member immediately.

5.) To acknowledge the Concussion Policy, which prohibits any player who has suffered any apparent brain trauma from returning to play without a doctor’s release.

6.) To acknowledge the walker/ pick up form.

7.) To stay with the assigned staff member (coach).

8.) To not be alone and use the buddy system when using the restroom. 

9.) To not display aggressive behavior or put there hands on others.

10.) To not use abusive or offensive language/gestures as they are not tolerated.

11.) To use fair play by observing the Laws of the Game. 

12.) To show good sportsmanship to all.

13.) To have fun!

​Violations of any aspects of this Code of Conduct may result in a disciplinary action. This action may be in the form of a reprimand, suspension or expulsion from Casares Soccer Camp without refund or voucher.

Medical Form/ Emergency Contact Form 

 

Please print out and complete the Medical Form/ Emergency Contact Form. You can either bring the completed form with you the first day of camp or mail it by June 10th. You will not be allowed to participate in camp until this form is completely filled out and received by the Camp Director.

Report Child Abuse

​

At Casares Soccer Camps, we strive to create a safe environment for all. All adult staff members of Casares Soccer Camps are mandatory reporters of any known or suspected child abuse or misconduct. All reports must be made to the appropriate law enforcement authorities. Casares Soccer Camps does not tolerate retaliation of any kind. Any adult staff member or minor camper who makes a good faith report will not be subject to retaliation, including harassment, as a result of making a report. The SafeSport Act also includes qualified immunity for good faith reports. Reporting:

  1. Any issues should be brought to the Camp Directors attention immediately.

  2. Call local law enforcement 732-388-3434

  3. Reporting to the U.S. Center for SafeSport. The following allegations must be reported within 24 hours to the U.S. Center for SafeSport, at https://uscenterforsafesport.org/report-a-concern/ or 833-587-7233

  4. In NJ you should report suspected child abuse to the New Jersey Child Abuse Hotline at 1-877-652-2873.
